City/StateNorfolk, VAWork ShiftFirst (Days)Overview:<h1>Sentara Health Plans is hiring a Care Coordinator/ Licensed Practical Nurse (LPN) for LOCERI/LTSS Waiver - Remote in Virginia</h1>Status: Full-time, permanent position (40 hours) Standard working hours: 8am to 5pm EST, M-FLocation: This position is remote in Virginia only.Job responsibilities:<ul><li>Support the Case Management team by reviewing completed LOCERI (annual assessments) to ensure accurate scoring and that all required elements are completed. Maintain a productivity target of 12–15 reviews per day.</li></ul>Education<ul><li>Associate's degree in Nursing preferred.</li><li>Bachelor's degree in Health & Human Services-related field or related experience in lieu of education accepted.</li><li>Licensed Practical Nurse (LPN) or Bachelor's prepared health professional strongly preferred. </li></ul>Certification/Licensure<ul><li>LPN license in VA or Compact/Multistate preferred</li></ul>Experience<ul><li>3 years Managed Care experience REQUIRED</li><li>Case Management experience preferred</li><li>Long-Term Services and Supports (LTSS) waiver experience preferred</li></ul>Sentara Health Plans provides health plan coverage to close to one million members in Virginia.
